Researchers at the University of Iowa and Vanderbilt University tested three hearing aid service models and two technology tiers. The results challenge long-held assumptions about the costs and care related to hearing loss, providing clarity for millions of people facing this issue.

The three service models studied by the research team

  • Full audiology services (custom fitting, counseling, follow-ups) with two tiers of prescription HAs
    • Prescription hearing aids with advanced features
    • Prescription hearing aids with standard features
  • Limited audiologist support for over-the-counter (OTC) devices.
  • Fully self-managed OTC devices.

Why it matters

OTC hearing aids work, but audiologist-fitted models deliver superior results.

Adults using audiologist-guided prescription hearing aids reported clearer hearing and higher satisfaction. But over-the-counter (OTC) options, while less effective, still proved they’re a viable alternative for mild hearing loss.

By the numbers

  • 245 participants aged 55+ with no prior hearing aid experience.
  • Prescription hearing aids fitted and supported by audiologists scored a meaningfully higher difference on a key 1–5 outcome scale than OTC options.
  • Patients receiving audiology-fitted hearing aids reported higher satisfaction scores than OTC groups
  • Prescription hearing aids with advanced features showed no advantage over prescription hearing aids with standard features

Reality check

Price doesn’t equal performance. For example, advanced features like noise cancellation or wireless connectivity didn’t improve daily outcomes. Researchers noted that today’s lower-end prescription hearing aids often include features once considered premium, making them sufficient for many users.

Buyer beware: If you're considering OTC devices, carefully research the options so you thoroughly understand what you're purchasing, including return policy, technical support, and guarantee.

The takeaway

  1. Audiologist-fitted hearing aids produce superior results. Service and support are the distinguishing features — expertise matters.
  2. OTC models are effective for those prioritizing affordability.
  3. Unless you need specialized features, skip them.
